I have been trying to upload images on smart action field forms. On the popup window that asks to display image with crop or select options, if I click on “Use this image button” it closes the form with windows alert “The “Upload Image” form values will be lost, are you sure to continue?”

We are very sorry for this inconvenience.
We are aware of this issue and we are working on it. We’ll let you know as soon as it is fixed.
Meanwhile, if you click on cancel, the document should be loaded correctly and the form still open. This could be a workaround waiting for our bug fix.
