Smart action enum checkbox widget has disappeared

Observed behavior

We have a form that has an Enum of two checkboxes this has worked without any issues since we launched this form which was about a few months ago but now all of a sudden the checkboxes has disappeared and we have not pushed anything out recently

Expected behavior

checkbox should appear in the smart action form

We just rollbacked a few recent changes delivered yesterday.
Can you check again and let us know if things are running again on your end?

Thanks for the rollback, we had similar issues but with dropdown menus.
